Core Skills with the Primary Care Team
Community Health Workers (CHWs) and Family Fartners (FPs) are essential members of the care team, often serving as the bridge between a family and other members of the primary care team. Their unique ability to engage and support families is imperative in improving access and quality of care.
The following CHW role-focused modules will introduce a general tiered approach that CHWs and FPs can use when working with the majority of children and their families. After introducing this approach, participants will have the chance to apply practical skills to several case examples.
